**Decision Matrix Overview**

CobbleStone®’s Decision Matrix is a tool that can be used to assist users to perform actions within the system such as select the correct Record Type. The idea behind Decision Matrices is to develop a series of questions and answers and based off the answers provided and selected, a recommended ending response appears.

A System Admin user can create decision matrices. Navigate to **Manage/Setup** - **Surveys and Questionnaires** - **Decision Matrices Setup** for the setup page.

On the Decision Matrix List screen, an admin can **view** a list of any decision matrices already created. Click **Decision Matrix Form** for the needed line item to view and edit the existing matrix.

An admin can also click **Add a New Decision Matrix** to add a new matrix.
